How relevant is the above quote to this gif on a scale of 1 to 5 with 5 being highly appropriate to 1 being not relevant at all.

By the way, do you know that around 2.3 billion people lack proper sanitation facilities, and close to one billion people defecate in the open due to unavailability of proper toilets. India is a leader on that front with around 60 - 70% of that billion.

For that matter, there was a report that came out in the recent years pointing out open poop problem on public side walks, even in SF.

On a personal note, sometimes, even with the availability of fully functional public toilets, there are people out there, who prefer open defecation and urination, out of convenience, lack of civic sense, and due to their prior experience with open defecation.

What is the big deal?
This increases the risk on water quality, spread of diseases, and on the economic front, brings down the value of a place.

Just to give you an idea, diarrhoea related illnesses kill more than 700000 children under five around the globe every year.
